There are 14 items to the Khalfa HSQ for hyperacusis. Please choose your answer from the dropdown menu for each question and press SEND at the end. A copy of your answers will be sent to your email address. If nothing arrives in your Inbox, please check your spam/junk folders
Your answers need to be calculated so when I have done this, I will email you with your calculated measures. If you do not hear from me within 48 hours, it means I have not received your questionnaire. Please email me, forwarding your copy of your answers to measures@tinnituseprogram.org. This happens infrequently, but emails have been known to 'disappear' in to the ether!
